Thin Endometrium- causes and treatment

Types of embryo transfers that are used in infertility treatment in an IVF setting


1. Fresh Embryo Transfer: Fertilized Eggs cultured for 2 to 3 days and best embryos chosen for Embryo transfer.


Disadvantage- cannot select the best embryos. Have to transfer more embryos. Little guess work involved.

2. Frozen Embryo Transfer: Any healthy embryos not used during fresh embryo transfer on day 2/3/5 can be frozen and used later.

3. Blastocyst Embryo Transfer: 5-day embryos are transferred, referred to as a blastocyst

Advantage- selection of best embryo. Cant limit transfer to 1 or 2 embryos. So have more for freezing

Azoospermia and Testicular Biopsy



It is important for couples to visit a doctor when they are unable to have a child despite having a normal sex life after 1 year. The root cause of the problem may be due to health issues that lies with either the male or female partner. In rare cases both of them may have some sort of reproductive issues.

However, 1% of all infertile males may not have any sperm in their semen. This is termed azoospermia that affects the ability of the patient to procreate. Fertility experts can however treat azoospermia effectively and help the man to become a parent . Some of the treatment procedures advised to treat azoospermia include:-

1) Surgery to remove obstruction of the vas deferens and create an additional passage for the sperms
2) Microscopic Epididymal Sperm Aspiration (MESA)
3) Testicular Sperm Extraction (TESE) / Micro TESE
4) Percutaneous Epididymal Sperm Aspiration (PESA)
5) Testicular Biopsy / Percutaneous Testicular Biopsy

Testicular biopsy

Testicular biopsy is surgery to remove a piece of tissue from the testicles. The tissue is examined under a microscope.

How the Test is Performed

The biopsy can be done in many ways. The type of biopsy you have depends on the reason for the test.

The skin over the testicle is cleaned with a germ-killing (antiseptic) medicine. The area around it is covered with a sterile towel. A local anesthetic is given to numb the area.A small surgical cut is made through the skin. A small piece of the testicle tissue is removed. The opening in the testicle is closed with a stich. Another stitch closes the cut in the skin. The procedure is repeated for the other testicle if necessary.

Needle biopsy is also commonly done. The area is cleaned and local anesthesia is used, just as in the open biopsy. A sample of the testicle is taken using a special needle. The procedure does not require a cut in the skin.Depending on the reason for the test, a needle biopsy may not be possible or recommended.

How the procedure will Feel

There will be a sting when the anesthetic is given. You should only feel pressure or discomfort similar to a pinprick during the biopsy and not pain.

Why the testicular biopsy is Performed

The test is most often done to find the cause of male infertility. It is done when a semen analysis suggests that there is absent sperm and other tests have not found the cause. Ispermthus obtained from a testicular biopsy will be used to fertilize a woman’s egg in the lab. This process is called in vitro fertilization.

Normal Results

Sperm development appears normal. No cancerous cells are found.

What Abnormal Results Mean

Abnormal results may mean a problem with sperm or hormone function. Biopsy may be able to find the cause of the problem.

In some cases, the sperm development appears normal in the testicle, but semen analysis shows no sperm or reduced sperm. This may indicate a blockage of the tube through which the sperm travel from the testes to the urethra. This blockage can sometimes be repaired with surgery.

Other causes of abnormal results:

Risks

There is a slight risk for bleeding or infection. The area may be sore for 2 to 3 days after the biopsy. The scrotum may swell or become discolored. This should clear up within a few days.

Precautions

you will be advised to wear an athletic supporter for several days after the biopsy. In most cases, you will need to avoid sexual activity for 1 to 2 weeks.

Using a cold pack on and off for the first 24 hours may lessen the swelling and discomfort.

Keep the area dry for several days after the procedure.

Continue to avoid using aspirin or medicines that contain aspirin for 1 week after the procedure.

Endometriosis and Infertililty



How to overcome endometriosis-induced infertility

Tissue lining the uterus (Endometrium) grows and thickens during a menstrual cycle to receive a fertilized egg (implantation). Without a pregnancy, the lining sheds and is discharged with blood during a menstrual period. Endometriosis occurs when this tissue grows outside the uterus and the shedding process deposits blood and tissue in the abdomen.
As a result, scar tissue can form in the fallopian tubes, adhere to the ovaries and/or cause misalignment of the organs, preventing natural conception. Although some women have no discomfort even with severe endometriosis, others experience abnormal menstrual bleeding and pain, tenderness in the abdomen and pelvis, and/or painful intercourse.

 

Infertility is often the after effect once endometriosis has been diagnosed. It is a disorder in which in other areas of the pelvis the endometrial or uterine tissue is developed. In the ovary and fallopian tubes, as well as the pelvic region, the same tissue can invade. In the abdomen, it can very seldom be detected. The tissue undergoes similar changes during the menstrual cycle, which are marked by internal bleeding in patients with endometriosis. It could also be painful.

 


When you see the doctor after being unable to conceive spontaneously for one year, the fertility doctor will make sure to rule out this disorder. However, while the cases are uncommon, a few women may be able to get pregnant even with endometriosis.

US court allows citizenship for baby born through surrogacy

A federal appeals court has ruled against the Trump administration’s attempt to deny citizenship to one of two twins born overseas to a gay married couple.

The 9th Circuit Court of Appeals in San Francisco concluded on Friday that an earlier decision by a Los Angeles trial judge should stand and that four-year-old Ethan Dvash-Banks is an American citizen.

Ethan was conceived with the sperm of his Israeli biological father and born in Canada through surrogacy.

His other father is a US citizen, and a district court ruled in May 2019 that legally a child does not have to prove a biological relationship if their parents were married at the time of their birth and that Ethan should be issued a passport.

The State Department appealed leading more than a dozen Democratic senators to write to secretary of state Mike Pompeo condemning an action that would result in family separation.

Poly cystic ovaries and its Effects on the female Anatomy.




Poly cystic Ovary Syndrome or PCOS is a condition that maybe hereditary.

PCOD(polycystic ovarian disease)

Is one of the commonest causes of infertility. Patients have multiple small cysts in their ovaries that occur when the regular changes of a normal menstrual cycle are disrupted. The ovary is enlarged, and it produces excessive amounts of androgen and estrogenic hormones.

Polycystic ovary syndrome (PCOS), also called hyper-androgenic anovulation (HA), or Stein–Leventhal syndrome, is a set of symptoms due to a hormone imbalance in women. Signs and symptoms of PCOS include irregular or no menstrual periods, heavy periods, excess body and facial hair, acne, pelvic pain, trouble getting pregnant, and patches of thick, darker, velvety skin. Associated conditions also include type 2 Diabetes, Obesity, Obstructive Sleep Apnea, Heart Diseases, Mood Disorders, and Endometrial Cancer.

PCOS is due to a combination of genetic and environmental factors. Risk factors for PCOS include obesity, not enough physical exercise, and a family history of someone of this condition. Diagnosis is based on two of the following three findings: no ovulation, high androgen levels, and ovarian cysts. Cysts are detectable by ultrasound. Other conditions that produce similar symptoms include adrenal hyperplasia, hypothyroidism, and hyperprolactinemia.

 PCOS has no permanent cure. Treatment for PCOS involves lifestyle changes such as weight loss and exercise. Birth control pills are used to help in regularizing periods, excess hair, and acne. Metformin and anti-androgens are also useful. Efforts to improve fertility include weight loss for obese patients, and administration of clomiphene, or metformin. In vitro fertilization is used in few cases wherein the treatment is not effective.

PCOS is the most common endocrinal disorder among women between the ages of 18 and 44. It affects approximately 5% to 10% of this age group. It is one of the leading causes of poor fertility.
